The following items are required to enroll your child at EHA

  (one of each per child, please):

• Application for Enrollment
  

• History and Physical Report Form
  

• Complete immunization record (obtain from your physician)
  

• If your child will be transferring from another school, please sign the Records Release Form and provide to the current school
  

• Emergency Information Card
  

• Parental Consent Form
  

The following forms are optional:

• After School Activities Form (click <HERE>)
  

• Lunch Forms (ongoing, check with office or click <HERE>)
  

• Milk Purchase Form
  

• Financial Aid Forms, if needed (available at sss.nais.org; use EHA school code #1659)
